Operative gynaecology is a narrow field that uses surgical methods of diagnosis and treatment of various diseases of the female reproductive system.

Diagnostic methods used in operative gynaecology
Laboratory and instrumental diagnostic methods do not always provide sufficient information about the state of a woman's reproductive system.
In such cases, surgical gynaecology offers two methods: diagnostic hysteroscopy and diagnostic laparoscopy.

Diagnostic hysteroscopy
During a diagnostic hysteroscopy, the cervical canal of the cervix is examined.
For this purpose, a special endoscopic device, the hysteroscope, is used.
Modern diagnostic hysteroscopes have a minimum diameter of 3-6 mm, so they often do not require dilatation of the cervical canal, which significantly reduces the traumatic nature of the examination.
Diagnostic hysteroscopy can be used to:

Evaluate the uterine cavity and cervical canal, the endometrium and the uterine tubal orifices.

Diagnose endometrial polyps, uterine cavity septum, some types of uterine myoma.

Identify and evaluate the presence and severity of adhesions, hysteroscopic signs of adenomyosis, endometrial hyperplasia and chronic endometritis.
During manipulation, the doctor can perform a targeted biopsy of tissue with further histological examination.
Diagnostic laparoscopy
During diagnostic laparoscopy, the doctor can evaluate the pelvic and abdominal organs.
A different access is used for this purpose.
Several small incisions are made in the anterior abdominal wall, through which the video camera and the necessary instruments are inserted.
Diagnostic laparoscopy creates an opportunity to visually assess the reproductive organs.
This method of research allows to detect adhesions in the pelvis, evaluate the uterus and its appendages, the patency of the fallopian tubes, the presence and prevalence of foci of endometriosis, diagnose malignancies.
Both hysteroscopy and laparoscopy can be performed for both diagnostic and therapeutic purposes, which is often performed as part of a single intervention.
That is, if during diagnostic hysteroscopy the doctor detected any pathology, it is possible to perform an operative procedure immediately.
For example, if a polyp is detected in the uterine cavity, the gold standard is to remove it; depending on the clinical case, an adhesion dissection or, for example, removal of a uterine myoma can be performed.
